- People whose personal information was stored in CUSO Financial Services' systems and impacted in the cybersecurity incident at a third-party service provider between December 19, 2023 and January 19, 2024 (notice sent October 2024). Includes a California Subclass.

- Cash payment of up to $599 (pro rata; may be less) and/or up to $5,000 for documented losses; California Subclass members may also claim up to $100; two years of credit monitoring

- Only for some benefits. Documentation needed for documented losses and, for the California payment, proof of California residence on December 19, 2023. Online claims use the Claim Number and PIN from the mailed notice
